Few diagnoses strike fear into people quite like cancer. Yet among the major cancers affecting Americans, colorectal cancer is unusual because screening does more than find disease early. It can prevent many cancers from developing in the first place.
Most colorectal cancers develop slowly from abnormal growths called polyps. During a colonoscopy, physicians can locate these growths and remove them before they have the opportunity to become invasive cancer. That creates a long window in which a person who feels completely healthy can take action that may save his or her life years later.
The central problem is that early colorectal cancer may produce no symptoms. Waiting until something feels wrong can allow a preventable or highly treatable disease to advance. For veterans—especially those with complicated medical histories, toxic-exposure concerns, gaps in preventive care, or other risk factors—understanding the screening process is an important part of long-term readiness.
What Colon Cancer Is
The colon and rectum make up the large intestine, the final portion of the digestive tract. The colon absorbs water and electrolytes from digested material, helps form stool, and moves waste toward the rectum for elimination. Colorectal cancer begins when cells in the inner lining of the colon or rectum acquire genetic changes that allow them to grow and divide without the normal controls that regulate healthy tissue.
Many colorectal cancers begin in gland-forming cells and are classified as adenocarcinomas. The process often starts with a polyp—a growth that projects from, or spreads along, the intestinal lining. Over time, some polyps accumulate additional cellular abnormalities, progress from low-grade changes to advanced dysplasia, and eventually invade deeper layers of the bowel wall.
Once cancer penetrates the bowel wall, it can spread into nearby lymph nodes or travel through the bloodstream to organs such as the liver or lungs. This is why the stage at diagnosis matters so much. A small localized cancer can often be removed surgically, while metastatic disease generally requires more extensive treatment.
How Colon Cancer Can Present
Early colorectal cancer frequently causes no noticeable symptoms. When symptoms do occur, they vary depending on the tumor’s location, size, whether it is bleeding, and whether it is narrowing or obstructing the bowel.
Bleeding Signs
Bright red blood, maroon stool, black or tarry stool, or blood detected only through laboratory testing.
Bowel Changes
Persistent diarrhea, constipation, narrower stool, altered frequency, urgency, or a feeling of incomplete emptying.
Systemic Signs
Unexplained fatigue, weakness, weight loss, reduced appetite, or iron-deficiency anemia.
Abdominal Signs
Cramping, bloating, persistent pain, pressure, nausea, vomiting, or symptoms of bowel obstruction.
Right-sided colon cancers may bleed slowly without producing visible blood. In some patients, unexplained iron-deficiency anemia, shortness of breath, weakness, or unusual fatigue is the first clue. Left-sided tumors are more likely to change stool caliber or bowel habits because that portion of the colon has a narrower passage and more solid stool.
Rectal bleeding is often blamed on hemorrhoids, especially in younger adults. Hemorrhoids are common, but their presence does not rule out another source of bleeding. Persistent bleeding, a new change in bowel habits, unexplained anemia, abdominal pain, or unexplained weight loss deserves medical evaluation regardless of age.
Risk Factors
Age remains an important risk factor, but colorectal cancer is increasingly diagnosed in adults younger than 50. Other risks include a personal history of colorectal polyps or cancer, a first-degree relative with colorectal cancer or an advanced polyp, inherited syndromes such as Lynch syndrome or familial adenomatous polyposis, and long-standing inflammatory bowel disease.
Modifiable factors associated with increased risk include smoking, heavy alcohol use, obesity, physical inactivity, type 2 diabetes, and diets high in processed meat or red meat and low in fiber-rich foods. A risk factor does not guarantee cancer, and a person without obvious risk factors can still develop it. Screening recommendations therefore apply broadly to average-risk adults, not only to people who feel vulnerable.
When Screening Should Begin
Major U.S. guidelines recommend that average-risk adults begin colorectal cancer screening at age 45. Screening generally continues through age 75. For adults ages 76 through 85, the decision is individualized based on prior screening, overall health, life expectancy, and personal preferences.
Earlier evaluation may be necessary for people with symptoms, inflammatory bowel disease, hereditary cancer syndromes, previous advanced polyps, or a strong family history. A common clinical approach for someone with an affected first-degree relative is to begin colonoscopy at age 40 or 10 years before the relative’s age at diagnosis, whichever comes first, although the exact recommendation depends on the family history and current guideline used by the clinician.
Mail-In and At-Home Screening Options
Stool-based tests allow patients to collect a small sample at home and return it by mail or to a laboratory. They are designed primarily for average-risk people who do not have symptoms. They are not the right tool for investigating ongoing rectal bleeding, iron-deficiency anemia, a suspicious mass, or other warning signs.
Fecal Immunochemical Test
The fecal immunochemical test, commonly called FIT, looks for human hemoglobin in stool. Because it is designed to detect blood from the lower gastrointestinal tract, it is more specific for colorectal bleeding than older guaiac-based stool tests. The patient typically uses a sampling device to collect a small amount of stool and mails or returns the kit according to the instructions.
FIT is inexpensive, does not require sedation or bowel preparation, and can be completed privately at home. Its greatest weakness is that it must generally be repeated every year. Polyps and cancers may bleed intermittently, so one negative test does not guarantee that no significant lesion is present. FIT also cannot show where bleeding came from or remove a polyp.
Stool DNA-FIT Testing
Multitarget stool DNA testing combines a test for hidden blood with analysis for abnormal DNA markers shed by colorectal cancers and some advanced polyps. It is usually performed less frequently than FIT, often every one to three years depending on the product and guideline.
The advantages are convenience, no sedation, and no bowel preparation. The disadvantages include higher cost, a larger stool sample, and more false-positive results than FIT alone. A positive stool DNA test does not diagnose cancer. It means the patient needs a colonoscopy to identify the cause.
The Central Rule for Home Screening
Every abnormal stool-based screening result must be followed by a timely diagnostic colonoscopy. Repeating the stool test to see whether it becomes negative is not an adequate substitute. The value of a home test depends on completing the full screening pathway.
Home-Test Advantages
No sedation, no driver, no bowel preparation, lower upfront burden, and easier access for many patients.
Home-Test Limits
Must be repeated regularly, can miss nonbleeding lesions, cannot remove polyps, and requires colonoscopy after a positive result.
Why Colonoscopy Is the Definitive Examination
Colonoscopy is often called the gold-standard or definitive colorectal examination because it allows a trained physician to inspect the rectum and the entire colon directly. It is not merely a detection test. It is also a diagnostic and therapeutic procedure.
During colonoscopy, a gastroenterologist can identify inflammation, bleeding, diverticular disease, masses, strictures, and polyps. Suspicious tissue can be biopsied, and most polyps can be removed during the same procedure. That ability to find and eliminate precancerous lesions is the primary reason colonoscopy can prevent cancer.
Colonoscopy is also the required follow-up after an abnormal FIT or stool DNA test. It is generally preferred for patients with symptoms, a history of polyps, significant family history, inflammatory bowel disease, or other high-risk conditions.
Preparing for a Colonoscopy
The physician needs a clear view of the colon lining. Even small amounts of stool can hide a flat polyp or other abnormality. The bowel preparation is therefore not a formality; it directly affects the accuracy and safety of the examination.
Several days before the procedure, the medical team may provide medication instructions. Blood thinners, diabetes medications, iron, and certain weight-loss or diabetes drugs may require individualized planning. Patients should not stop prescribed medication on their own. The prescribing clinician and endoscopy team should decide how it will be handled.
Many patients are instructed to limit high-fiber foods for a period before the procedure and then transition to clear liquids the day before. Clear liquids may include water, clear broth, sports drinks, apple juice, tea or coffee without milk, and gelatin that avoids restricted colors. Instructions vary, and the patient should follow the specific preparation sheet provided by the endoscopy center.
Modern preparation commonly uses a split-dose regimen. The patient drinks part of the laxative solution the evening before the examination and the remainder several hours before arrival. Split dosing generally cleans the colon better than taking the entire preparation the night before. Frequent watery bowel movements are expected, and patients should remain near a bathroom, stay hydrated with approved liquids, and use skin-protection products if irritation develops.
What Happens on Procedure Day
At the endoscopy center or hospital, the patient changes into a gown and reviews the medical history, medication list, allergies, and preparation quality with the clinical team. An intravenous line is placed for fluids and medication. Vital signs are monitored throughout the process.
Sedation practices vary. Many U.S. colonoscopies use moderate sedation or monitored anesthesia care, often with propofol. Some patients are deeply asleep, while others remain very relaxed and drowsy. Because sedating medications impair judgment, coordination, and reaction time, the patient needs an approved adult to provide transportation home.
The colonoscope is a long, flexible instrument with a light, high-definition camera, water irrigation, suction, and channels through which miniature tools can be passed. The lubricated scope is inserted through the anus and advanced carefully through the rectum and colon, usually to the cecum, where the large intestine begins near the appendix.
Carbon dioxide or air is introduced to gently expand the colon so the physician can see between folds. Carbon dioxide is absorbed rapidly by the body and may reduce post-procedure bloating. The physician examines the colon during insertion, but the slow withdrawal phase is especially important because this is when the lining is inspected carefully for subtle abnormalities.
If a polyp is found, the physician may remove it using forceps, a wire snare, electrocautery, cold-snare technique, or a more advanced endoscopic resection method. A suspicious mass may be biopsied. Photographs are commonly taken to document landmarks and findings. Procedure time varies, but an uncomplicated examination often takes approximately 20 to 45 minutes.
Recovery After Colonoscopy
After the scope is removed, the patient is monitored in a recovery area while the sedation wears off. Temporary bloating, gas, or mild cramping is common. Patients are usually discharged the same day and may return to a normal diet unless the physician provides other instructions.
Patients should receive written guidance explaining warning signs. Severe or worsening abdominal pain, persistent vomiting, fever, dizziness, heavy rectal bleeding, black stool, fainting, or other concerning symptoms require prompt medical attention. A small amount of blood may occur after polyp removal, but the endoscopy team should explain what is expected in the individual case.
Dispelling Common Colonoscopy Myths
Myth: The procedure is extremely painful. Most patients are sedated and experience little or no memory of the examination. Discomfort is more commonly associated with the bowel preparation or temporary gas afterward than with the scope itself.
Myth: Everyone has to drink a full gallon of terrible liquid. Preparation options now include several volumes and formulations. Not every option is appropriate for every patient, especially those with kidney, heart, or electrolyte problems, but clinicians can often choose a regimen that balances effectiveness and tolerability.
Myth: A positive home test means cancer. Benign polyps, hemorrhoids, inflammation, and other sources of blood can produce a positive result. Colonoscopy is needed to determine the cause.
Myth: No symptoms means no cancer. Screening exists precisely because early cancers and advanced polyps often produce no symptoms.
Myth: Colonoscopy spreads cancer or makes it worse. Standard biopsy and polyp-removal techniques are established parts of cancer diagnosis and prevention. The benefits of obtaining an accurate diagnosis greatly outweigh the theoretical fears often circulated online.
What Polyps Are and What They Can Mean
A polyp is a growth arising from the colon’s inner lining. Some are attached by a stalk, while others are broad, flat, or difficult to distinguish from surrounding tissue. Size, number, location, shape, microscopic type, and cellular features all affect future risk.
Hyperplastic polyps in the lower colon are often low risk. Conventional adenomas—tubular, tubulovillous, or villous—are precancerous. Sessile serrated lesions and traditional serrated adenomas can also lead to cancer through a different molecular pathway and may be subtle or flat.
Pathologists examine removed tissue under a microscope. They determine the polyp type, measure important features, look for dysplasia, and assess whether cancer is present. Most polyps are not cancer. Finding and removing a precancerous polyp means the procedure accomplished exactly what screening is designed to do.
How Polyps Are Removed
Very small polyps may be removed with cold biopsy forceps, although cold-snare removal is commonly preferred for many small lesions because the wire loop captures a rim of surrounding tissue. Larger polyps may require a hot snare using electrocautery, injection of fluid beneath the lesion, placement of clips, or advanced procedures such as endoscopic mucosal resection.
Most removals do not require an external incision. The instruments pass through the colonoscope. If a polyp is extremely large, deeply invasive, located in a difficult area, or cannot be safely removed endoscopically, surgery or referral to an advanced endoscopist may be necessary.
When the Next Colonoscopy Is Due
Follow-up timing depends on the quality and completeness of the first examination and the pathology findings. A high-quality normal colonoscopy in an average-risk adult is commonly repeated in 10 years.
Current post-polypectomy guidance generally recommends a seven-to-10-year interval after removal of one or two small tubular adenomas with low-risk features. Three or four small adenomas commonly lead to a three-to-five-year interval. Five to 10 adenomas, an adenoma at least 10 millimeters, villous or tubulovillous features, or high-grade dysplasia commonly lead to a three-year interval.
More than 10 adenomas may require follow-up in approximately one year and consideration of genetic evaluation. A large lesion removed piecemeal may require a repeat examination in about six months to ensure that no residual tissue remains. Serrated lesions have their own surveillance intervals based on number, size, location, dysplasia, and confidence in complete removal.
These intervals assume a complete, high-quality examination with adequate bowel preparation and complete removal of identified lesions. Poor preparation, incomplete advancement of the scope, uncertain resection, family history, symptoms, or other clinical factors can shorten the interval. The patient should rely on the written recommendation from the treating gastroenterologist rather than a generic schedule.
Colon Cancer, Military Service, and Toxic Exposure
Veterans may have encountered burn-pit smoke, fine particulate matter, diesel exhaust, petroleum products, solvents, pesticides, industrial chemicals, radiation, contaminated water, and other hazards. Some military exposures are known to increase the risk of particular cancers. For colorectal cancer, however, the evidence varies by exposure and is still developing.
It is important not to overstate what science has proven. A veteran’s history of deployment or toxic exposure does not automatically establish that a later colon cancer was caused by service. At the same time, incomplete evidence is not the same as evidence of no risk. Exposure research often takes years, cancers may have long latency periods, and individual veterans may have experienced combinations of hazards that are difficult to reconstruct.
Veterans should document where they served, the dates and duration of deployments, known exposure incidents, occupational specialties, protective equipment used, and any persistent gastrointestinal symptoms. That information can help clinicians evaluate the whole risk profile and may become important if scientific evidence or VA policy changes.
Additional Veteran Risk Factors
Toxic exposure is only one part of the veteran-health picture. Tobacco use, alcohol misuse, obesity, reduced physical activity after separation, chronic stress, disrupted sleep, post-traumatic stress symptoms, depression, food insecurity, and delayed preventive care can influence cancer risk directly or indirectly.
Operational environments may normalize irregular meals, processed foods, energy-dense rations, tobacco, and sleep deprivation. After service, chronic pain or mobility limitations can make exercise difficult. Mental-health symptoms may interfere with appointments, preparation instructions, or follow-through after an abnormal test. None of these factors reflect a lack of discipline; they are practical barriers that health systems and veteran organizations should help people overcome.
VA Screening and Toxic-Exposure Resources
Veterans enrolled in VA health care can discuss colorectal screening with their primary-care team. Depending on age, symptoms, risk, medical history, and local availability, the VA may offer stool-based screening, colonoscopy referral, diagnostic evaluation, and follow-up care.
The VA Toxic Exposure Screening is designed to identify possible military environmental exposures and connect veterans with additional information or evaluation when appropriate. Veterans can also request a Military Environmental Exposure Assessment through a VA environmental-health clinician for a more detailed review of their exposure history and health concerns.
Exposure registries—including those related to airborne hazards and open burn pits, Agent Orange, Gulf War service, ionizing radiation, and contaminated water at Camp Lejeune—may help eligible veterans document service history and contribute information to ongoing research. A registry examination is not the same as filing a disability claim, and participation does not by itself establish service connection.
Veterans who develop colorectal cancer and believe military service contributed should consider speaking with an accredited Veterans Service Organization representative, VA-accredited claims agent, or VA-accredited attorney. Eligibility and service connection depend on the law, medical evidence, service records, and the individual facts of the case.

The Bottom Line
Colorectal cancer remains a major cause of cancer death, but it is also one of the cancers most vulnerable to prevention. A stool test can identify people who need further evaluation. A colonoscopy can reveal the source of bleeding, diagnose cancer, and remove precancerous polyps during the same examination.
The preparation may be inconvenient. The subject may feel embarrassing. The procedure may require taking a day away from work and arranging a ride. Those are real obstacles—but they are temporary. An advanced cancer diagnosis can alter the rest of a person’s life.
For veterans, staying current with colorectal screening is part of protecting the mission after service. Document exposures, report symptoms, complete recommended follow-up, and do not allow embarrassment or outdated myths to delay care. Early detection saves lives, and removal of a precancerous polyp can prevent a battle that never needed to be fought.
